How to Compare FQHCs: A Decision Framework

What actually matters when choosing a community health center.

Start With Your Needs

Before comparing clinics, clarify what you need:

  • Ongoing primary care, or a one-time urgent visit?
  • Specific services (dental, mental health, prenatal, pediatric)?
  • Language preference or need for interpretation?
  • Transportation constraints — how far can you realistically travel?
  • Schedule constraints — evenings or weekends required?

Evaluation Checklist

Access & Logistics

  • Hours include evenings or weekends?
  • Walk-ins accepted, or appointment only?
  • Distance from home or work?
  • Public transit accessible?
  • Telehealth available?
  • Wait time for new patient appointment (call to ask)?

Services

  • On-site dental (not just referral)?
  • On-site behavioral health?
  • On-site pharmacy or 340B program?
  • Prenatal care if needed?
  • Pediatric care if needed?
  • Specialist referral network established?

Language & Culture

  • Bilingual staff available?
  • Interpreter services in your language?
  • Culturally competent care for your background?
  • Patient materials in your language?

Quality Signals

  • HRSA Health Center Quality Leader designation?
  • CHQR badges relevant to your conditions?
  • Patient reviews (look for patterns, not just overall score)?
  • How long has the clinic been operating?
  • Is it FQHC or FQHC Look-Alike (both are good)?

Cost & Insurance

  • Accepts your insurance?
  • Sliding fee scale available if uninsured?
  • Enrollment help available for Medicaid/CHIP?
  • On-site financial counselor?

Questions to Ask When You Call

  • "Do you accept new patients?"
  • "How long is the wait for a new patient appointment?"
  • "Do you offer same-day or walk-in appointments?"
  • "Do you have dental / mental health / pharmacy on-site?"
  • "Do you have staff who speak [language]?"
  • "I'm uninsured — do you have a sliding fee scale?"
  • "Do you have someone who can help me apply for Medicaid?"
